FIAC Application Submission


Remember that submission of a digital FIAC is only a subset of the application process, and will not guarauntee the Board review of an application. View the rest of the submission steps here.


Keep the following in mind as you complete your application:

  • Please format your attachments to include page numbers. You should reference a particular attachment's page number in a short note on the corresponding section of the FIAC Application.
Attach addendums for the following sections where needed. Only the sections marked as (Required) are necessary to submit an application.

1. Attach your completed FIAC Application (Required)



2. Attach the Corporate Document sections of your application (§6-7 of the FIAC)



3. Attach the Business Profiles sections (§8 and 9) (Required)



4. Attach the Affiliate Enterprise sections (§10, 11, and 12)



5. Attach the Proof of Financial Responsibility (§13) (Required)
  • Do not attach your Financial Statement from section §13 d) i. here. Instead, upload it to the following section (#6 in the Submission Portal).
  • Insurance companies should append their annual statement and certificate to this section.


6. Attach a Financial Statement from §13 d) i. of the FIAC Application (Required)



7. Attach a Business Proposal (§14) (Required)



8. Attach an optional attachment for the FIB to review (§15)
